OVERVIEW OF OUR 2007 FUND RECIPIENTS

2008 APPLICATION DEADLINES
Applications for system innovation funds will be accepted four times in 2008:

  • Wednesday, January 2
  • Wednesday, April 2
  • Wednesday, July 2
  • Wednesday, October 1

OVERVIEW

  • System innovation funds are used to promote innovative program ideas that cannot be funded by other sources, but could result in the development of a new service delivery strategy.
  • System innovation funds are one-time only awards and will not continue beyond the expiration date of the funded proposal. Responses to the continuation/sustainability of the services will be reviewed carefully.
  • System innovation funds cannot be used for capital or start-up expenses.
  • Applicants must be able to explain how their proposed program will produce specific positive outcomes for consumers in our system of care.
  • Program ideas must be consistent with our Strategic Business Plan, ODADAS Community Plan, ODMH MSPA, and Needs Assessment.
  • Applicants seeking funds for prevention must agree to use the Rensselaerville framework (also known as TARGETrak).

GENERAL INSTRUCTIONS

  • Submit a standardized application and budget.
  • Applicants do not have to be a current ADAMH contract provider. However, they will have to agree to the ADAMH contract specifications if their application is approved by the ADAMH Board of Trustees.
  • Answer each question to the best of your ability using data to support your responses whenever possible.
  • Limit your submission to no more than five pages. Quality of answers will be evaluated versus quantity.
  • Email the completed application and budget to Joe Florenski, Grants Coordinator.

REVIEW PROCESS

  • Applications will be reviewed by a committee of ADAMH staff, consumers and/or family members, and community content experts. Funding recommendations will be submitted to ADAMH's CEO and the Board of Trustees for approval.
  • Applicants will be notified of funding decisions after the appropriate board meeting.
  • Fund recipients will submit quarterly reports on a standard template.
