Data Submission
Since the inception of the Outcomes Initiative in January, 2000, we have received over 77,000 outcomes assessments thanks to the concerted efforts of our service providers. The graph below demonstrates our success in the implementation of this process.

Quality Improvement
In Franklin County, we are committed to continuous quality improvement in all areas of mental health and alcohol and drug treatment. In tandem with the growing emphasis that state and federal agencies have placed on the importance of client outcomes, we have developed rigorous data collection, import, and analysis protocols to ensure the highest standards of data integrity and reliability are maintained. At each juncture on this continuum, our SAS-based system generates provider-specific and system-wide reports that are distributed monthly. This feedback loop is essential as it serves to inform and encourage providers about the quantity and quality of their data. It is upon this substrate of superior data that we can confidently develop analyses that explore best practices models of client recovery.
ADAMH has just released its 2003 Summary Summary Report that captures the essence of outcomes assessment administration and data submission and ranks provider participation in the outcomes initiative. ERROR REPORTS
Outcomes assessments are administered and entered by each provider and subsequently transferred electronically to ADAMH, either by diskette or secure bulletin board (BBS). Client identity is validated by comparing client demographics to the ODMH members database, and algorithms expunge any assessments having erroneous data. Error reports are mailed to providers monthly, and providers are requested to return these reports to ADAMH with the necessary corrections.

SUMMARY ERROR REPORTS
As error reports are cumulative, there is a tendency for some errors to "stagnate" for months or years and avoid correction. This system-wide report facilitates timely error correction, by listing for each provider the percentage of errors to good assessments received, as well as the percentage of errors lingering more than six months. This maximizes provider effort, by ensuring that time spent administering and entering assessments is not wasted.

MISSING DATA REPORTS
In order for data to be useful, it must not only be accurate but also complete. These provider-specific reports list the frequency that individual questions and composite scores from outcomes forms are omitted. Providers are encouraged to periodically examine these reports and to investigate possible causes of and solutions to missing data for those items most often omitted.

SUMMARY MISSING DATA REPORTS
While provider-specific missing data reports elucidate to providers where (e.g., on which items of the form) they excel or improvement may be needed, this system-wide report gives a global incompletion rate for each type of outcomes form. This allows providers to compare their form completion rates to each other and to the system as a whole.

SUBMISSION REPORTS
These cumulative, provider-specific reports display the type (e.g., assessment form) and number of assessments that were administered and received by ADAMH every month. Subsidiary reports group these assessments by intake and follow-up/in-tx and also list the number of assessments administered each month that contain errors still requiring correction.

SUMMARY SUBMSSION REPORTS
This non-cumulative, system-wide report displays the type (i.e., assessment form) and number of assessments that were administered by each provider and received by ADAMH every month. It is useful in comparing across providers the anticipated lag that occurs between form administration and submission to ADAMH.

MAPPING REPORTS
Assessments may be accurate and complete but still lack utility if they are not administered consistent with established ADAMH and ODMH methodologies. These provider-specific reports cumulatively list all treatment episodes for individual clients as per MACSIS claims. Outcomes assessments are superimposed over this matrix, allowing providers to compare specific outcomes assessments ADAMH expected to those that were administered.

PRODUCTION REPORTS
While mapping reports wield a tremendous amount of information, they are also cumulative, cumbersome, and contain client-identifying information. Provider-specific production reports are a more user-friendly approach, and summarize provider mapping reports at monthly intervals. This allows providers to monitor their administration rates for each assessment form type, and to compare these rates to those expected by ADAMH.

SUMMARY PRODUCTION REPORTS
As admission and administration rates often fluctuate from one month to the next, production reports should not be used to compare providers to each other at any single point in time. The summary production report however, smoothes out these wrinkles, by incorporating quarterly not monthly intervals. This allows providers to compare their assessment completion rates to each other and to the system as a whole.

SUMMARY SUMMARY REPORT
The many and varied outcomes QI reports can seem like a formidable obstacle to those unfamiliar with their uses and caveats, thus necessitating a single, comprehensive report. The summary summary report condenses six critical domains from the summary error report, summary missing data report, and summary production report and ranks providers in order of participation in the outcomes initiative. This clear and concise global picture most readily answers the provider who asks, "So…how are we doing?"
